Support During Life Changes

If you’re leaving the hospital, coming home from treatment, or transitioning from a facility or jail, we’re here to help. We make sure you have what you need, rides, medication help, follow-up appointments, or just someone to check in with you.

Connecting To Services

We help you get connected to resources like housing help, food programs, transportation, tribal services, traditional healing, and more—whatever helps support your well-being and goals.

Whole-Person Wellness Planning

We take time to listen and learn about your health, needs, and goals. Together, we create a plan that works for you, honoring your voice, culture, and what matters most to you and your family.

Health Education & Encouragement

We provide simple tools and guidance to help you stay on top of your health, like learning about your medications, chronic conditions, or ways to live a healthier, balanced life. We meet you where you're at, without judgment.

Help Coordinate Care

We work with your doctors, mental health providers, tribal programs, healers, and anyone else on your care team to make sure everyone is working together and you’re not left navigating it all alone.

Family & Caregiver Support

We include your family or support circle, if you want, so they can understand how to help and feel supported too. We’re here for your loved ones, not just you.
